Identify Zoned School

Establish Parents’/Legal Guardians’ Identity

Parents and legal guardians must identify themselves by submitting one (1) form of picture identification.


In order for a child to be enrolled in Lexington/Richland District Five, they must reside in the district with a parent or legal/guardian or custody that has been awarded by the court, Department of Social Services, or a retainer from an attorney stating pursuit of guardianship.


Who can register a child for school?

  • A parent (natural or adoptive); Step-parents may not register a child for school
  • A legal guardian with a court-issued guardianship order
  • A foster parent with proof of student placement from the South Carolina Department of Social Services
  • A sponsor with an approved International Exchange Program enrolling a child with a J-1 visa

Establish Residency

THREE acceptable pieces of documentation, which verifies their connection to the property.

Only one of the following is required

  • Property Tax Bill for the permanent residence
  • Mortgage Statement (current)
  • Rental Lease (complete copy, listing all occupants)
  • Real Estate Closing Documents (where applicable)


Two of the following are required

  • Vehicle Tax Registration
  • Voter Registration Card
  • Electric Bill
  • Water Bill
  • Telephone Bill
  • Cable/Satellite Bill


If you have recently moved and are unable to provide the two additional required documents listed above, you must, to a minimum provide a Rental Lease or Real Estate Closing Document.


Within 45 days of enrollment, you, as the parent or legal guardian, will be required to provide two documents of mail in your name that show the address of the person with whom your family resides. Junk mail is not acceptable.

    • Acceptable documents are:
      • paycheck stub
      • W-2 statement
      • car insurance
      • bank statement
      • state supported services
      • voters registration card
      • vehicle tax registration

IDs cannot be accepted as documentation for address verification.


When a parent is referred to the District Office for the state affidavit or a proof of residence concern they must satisfy the following:


Only the state affidavit or residency concerns will be done at the District Office. Contact the Office of Student Services and Planning for further information.

Birth Certificate

Legal Birth Certificate (long form) with seal.  A mother’s copy or a hospital copy is not acceptable.

Provide Proof of Immunization

All immunizations must be recorded on a SC Certificate of Immunization. 

Additional Documents

Custody/guardianship orders

  • Any court orders that prevent or limit access of a parent to the child or the child’s educational records.
  • Any restraining orders or legal documents that limit the access of any individual to the child being registered.


Medicaid Card (if applicable)


For Middle and High School (Grades 6-12)   A copy of your child's unofficial transcript (ideal) or latest report card from their previous school.

School District Five uses an online registration platform called FinalForms, which is open all year long. At any point in the year, you may log into FinalForms to register a new student or update student information like addresses, phone numbers, etc.
